IMPORTANT INFORMATION FOR PATIENTS
1. Face Masks are OPTIONAL. If you are experiencing ANY type of sick symptoms, WE WILL ASK that you wear a mask for the safety of other patients, staff, and yourself.

2. Patients ONLY unless they require assistance. Limit one person for assistance.

3. If being seen for possible COVID-19 symptoms DO NOT ENTER THE FRONT DOOR. Call the office from the parking lot when you arrive for your appointment to receive further instructions.
